山祖神社と刻まれていますので、この石碑自体かあるいは近隣のクヌギの木立が御神体かと思われます。
 It is a stone monument near Omitama City's home for the elderly, Respect Forest.
Since it is engraved with Sanso Shrine, it seems that this stone monument itself or the nearby oak grove is the sacred body.

★ ★ ★ ★ ★ 御祭神・御由緒とも分かりません。狭い道路脇，林の中に祀られた「山祖神社 昭和二十三年十月十五日」と刻まれた石碑のみの社です。社名からも見当のつけようもないのですが，石碑を囲った竹や縄の様子からしても今も祀られていらっしゃる様子が窺えます。
I don't even know the god of rituals or history. It is a shrine with only a stone monument engraved with "Sanso Shrine, October 15, 1945" enshrined in the woods on the side of a narrow road. I have no idea from the company name, but even from the appearance of the bamboo and rope surrounding the stone monument, it can be seen that it is still enshrined.
