最新の物は運転免許証同様のカード型です。
自分のようにしまい忘れて紛失する方もみえると思いますので簡単に再発行の手順を記載します。
①かつての同僚に連絡して修了書に記載の「指定教習機関」を確認
②指定教習機関に電話して自分の登録があるか確認してもらう。
③HPから再発行申請書をダウンロードし、プリントアウトして記入。
④写真と免許証(身分証明書)、記入した用紙とお金を持って訪問。
以上となります。
難点は駐車場が2か所しかないこと。敷地内に1か所、ブロック内北側1分のところに1か所のみ。よって☆1個減。ちなみに刈谷中央図書館の駐車場は徒歩5分です。
 We asked for a reissue of a forklift driving skill training certificate.
If you prepare a photo, you can reissue it on the same day.
The latest one is a card type similar to a driver's license.
Some people like me may forget it and lose it, so I will briefly describe the procedure for reissuing it.
① Contact a former colleague and confirm the "designated training institution" stated in the certificate
② Call the designated training institution and ask them to confirm that they are registered.
③ Download the reissue application form from the website, print it out, and fill it out.
④ Visit with a photo, driver's license (identification card), completed form and money.
That is all.
The difficulty is that there are only two parking lots. Only one on the premises and one minute north of the block. Therefore, ☆ 1 less. By the way, the parking lot of Kariya Central Library is a 5-minute walk.
