★ ★ ★ ★ ★ ２０１７年５月に自殺した兵庫県多可町立八千代小学校５年の女子児童＝当時（１０）＝は、いじめに苦しんでいることに、いくつかのサインを出していた。第三者委員会は、女児が「いびつな社会関係」の女子グループで仲間外れや蹴られるなど継続的な心身の嫌がらせを受けていたにもかかわらず、学校はＳＯＳを見逃し、積極的な関与ができなかったと問題視。自殺の予見は難しかったとしながら、組織的な対応の重要性を強調した。
自殺した直後、学校側は「女児からいじめの訴えはなかった」との見方を示していたが、ほかの児童から指摘があったほか、女児が４年時のアンケートで３回、いじめの有無を問われ、「はい」に○を付けてから消し、「いいえ」に○を付けた形跡があった。ストレス状態を測る年２回の「ストレスチェック」でも高いストレスへの移行がみられていた。
第三者委は、学校がアンケート結果などを生かせず、担任がほかの児童のケアに気を取られるなど表面的な対応にとどまったため、女児の苦痛をキャッチできなかったと指摘。見逃しの要因として、いじめの組織的対応が未整備▽前思春期の発達段階にある女子グループの理解不足▽学校の統廃合による教職員の多忙－の３点を挙げた。
