本社は旧社とのみ云い、創立の年月日は不詳である。或は云う延喜式神明帳に記せし衣太手神社の内の一社であると、旧号を五葉山本宮神倉野権現と称し、盛町神山家の祖和光院成光より二十二世の間同地に住して、建長二年（１２５０）より同社に奉仕せるを見れば鎌倉時代以前の創立なることが明らかである。太陰暦廃止以前は毎年六月二十七日を以て例祭日とし、明治以前は同村の外田茂山（現盛町）村、猪川村、赤崎村、大船渡村の総鎮守として毎年例祭には五カ村の役村が参拝されている。
